Several life changes have been identified as stressful processes for children and their families, as social and academic skills are ever changing during the course of the first school years. This is the reason why it is necessary for the school, family and community to cooperate. The schools (kindergarten - elementary) can support children’s transitional readiness through collaboration and by implementing policies and curriculums. School staff, parents and the state will by engaging into active collaboration play a crucial role in the transitional process and will guarantee its success. When the educational process is being interrupted it has been observed that children are put under stress which hinders them from developing social skills and
creates educational barriers. Children with special educational needs or some form of disability and their families are facing considerable problems when transitioning between different school environments throughout the first years of childhood. There are certain activities that would bridge the gap between a kid’s home environment,
kindergarten and primary school to provide a sense of continuity.
It is of the utmost importance that the kid’s development is closely
monitored by the social services, parental support services and by the preschool educational system in order to make sure that the best strategy is chosen for the kid given its unique set of needs

| Abstract: | Οι πολλές μεταβάσεις προσδιορίζονται ως αγχωτική κατάσταση για τα
παιδιά και τις οικογένειες καθώς οι κοινωνικές και ακαδημαϊκές δεξιότητες των παιδιών είναι συνεχώς μεταβαλλόμενες κατά τη διάρκεια των πρώτων σχολικών χρόνων, γι΄αυτό είναι απαραίτητη η συνεργασία μεταξύ σχολείου, οικογένειας και κοινότητας. Τα σχολεία (νηπιαγωγείο - δημοτικό σχολείο) μπορούν να ενισχύσουν
την ετοιμότητα των μικρών παιδιών, επιχειρώντας συνδέσεις μεταξύ των εκπαιδευτικών βαθμίδων και υιοθετώντας πολιτικές και προγράμματα σπουδών που διασφαλίζουν την ομαλή μετάβαση στο δημοτικό σχολείο. Οι εκπαιδευτικοί, οι γονείς, οι διευθυντές, τα στελέχη της εκπαίδευσης, η διοίκηση και οι φορείς δημοσίου με την ενεργό συνεργασία θα γίνουν πρωταγωνιστές που θα παίξουν σημαντικό ρόλο στη διαδικασία της μετάβασης και στη διασφάλιση της επιτυχίας της. Όταν υπάρχει ασυνέχεια στην εκπαιδευτική διαδικασία έχει παρατηρηθεί ότι δημιουργείται άγχος στο μικρό παιδί που το εμποδίζει να ρυθμίζει μόνο του τις κοινωνικές δεξιότητες και δυσκολεύεται να αντιμετωπίσει αποτελεσματικά τη μάθηση.
Τα παιδιά με ειδικές εκπαιδευτικές ανάγκες ή αναπηρία και οι οικογένειές τους αντιμετωπίζουν ιδιαίτερα προβλήματα στις μεταβάσεις σε πολλά περιβάλλοντα κατά τα πρώτα χρόνια της παιδικής ηλικίας Γι΄αυτό η παροχή συνέχειας μπορεί να γίνει μέσα από δραστηριότητες που γεφυρώνουν το χάσμα μεταξύ σπιτιού νηπιαγωγείου και δημοτικού σχολείου. Απαραίτητη είναι η παρακολούθηση της ανάπτυξης του παιδιού από τις
κοινωνικές υπηρεσίες, τις υπηρεσίες γονεïκής υποστήριξης και του συστήματος προσχολικής εκπαίδευσης μέσα από πρακτικές μετάβασης, ώστε να προσδιοριστούν οι στρατηγικές που θα υιοθετηθούν για τη βέλτιστη μετάβαση
όλων των παιδιών και των παιδιών με ειδικές ανάγκες, αφού ανιχνευτούν οι ιδιαιτερότητές τους.
